By submitting this form, I agree to the following: I am age between 12-22. I will perform at the Anglesea Music Festival between the 18th and 20th of October in the Secondary Marquee. My performance will not contain course language or any other material that may be offensive to viewers. My act will not exceed the time limit set by the event organisers I give permission for any photos taken of me at the event to be used in media. All performers and only performers will receive a free pass to the festival. No payment will be received for performing however you become part of the festival to showcase and promote your act. You may sell your merchandise at your performances or through the merchandise tent. BY AGREEING TO THE TERMS & CONDITIONS YOU ARE GIVING THE AMF COMMITTEE PERMISSION TO USE THE PHOTO, BIO, VIDEO, AUDIO & URL'S THAT YOU PROVIDE IN YOUR APPLICATION TO PROMOTE THE 2011 & FUTURE AMF EVENTS. YOU ARE ALSO GIVING THE AMF PERMISSION TO USE ANY PHOTO, VIDEO OR AUDIO TAKEN OF YOU DURING THE FESTIVAL FOR THE PURPOSE OF PROMOTIONS. I agree
